The Leyland Collaborative
We are pleased to be able to offer evening and weekend appointments by working in collaboration with other GP Practices in the area.
This does mean that if you want to be seen on a weekday evening or weekend, your appointment may be held at a different GP practice to your registered practice and with a different GP or nurse than you usually see.
The practices are working together and where you may be offered an appointment are:
SANDY LANE SURGERY, MOSS SIDE MEDICAL CENTRE, WORDEN MEDICAL CENTRE, CENTRAL PARK SURGERY, STATION SURGERY & THE VILLAGE SURGERIES (Croston and Eccleston).
Your medical records will be available to the clinician that sees you to ensure that they have the information they need to give you the best possible care.
In order to book an evening or weekend appointment, you must call this GP practice as these appointments are not currently available online.
However if you need to book an evening or weekend appointment and your GP surgery is closed, please call NHS 111 and they will be able to advise you further.
* Under the increased access scheme and as part of the Leyland collaborative we will open some weekday evenings from 18:30 - 20:00*
Your GP or Nurse can only usually deal with one problem per appointment, this is so we can deal fully with your problem and complete all documentation afterwards. This helps us to run to time and avoid inconvenience for other patients, please dont be upset if we ask you to re-book if you wish to discuss other problems.
Extended hours will be covered by different GPs on a rota basis. Some Doctors have other NHS commitments, including education, training and NHS management roles.
